When a participant engages with United Support Services for NDIS support, they will have their first meeting with our Engagement Manager. During this meeting, the Engagement Manager will assess the participant's NDIS plan and discuss how we can best support them in achieving their goals. To better understand the participant's needs, the Engagement Manager will encourage them to share information about themselves, such as their name, NDIS number, disability, and important people in their life.
At this stage, a Service Agreement will be developed to outline how United Support Services and the participant will work together. If the participant chooses United Support Services as their provider for Coordination of Supports, they will need to sign the Service Agreement. The Engagement Manager will then match the participant with the most suitable Support Coordinator to meet their unique needs, goals, and aspirations.
Following this, the participant's dedicated Support Coordinator will get in touch and start their NDIS journey together.
Once the participant is matched with their Support Coordinator, the Coordinator will arrange a meeting with them and their circle of support. This meeting is intended to gather a comprehensive understanding of the participant's life and identify the best approach to achieve their NDIS goals.
During the meeting, the Support Coordinator will discuss the funded support categories, explain the guidelines for Support Coordination, and work with the participant to develop a personalized and comprehensive program that reflects their specific needs, desires, and aspirations. This tailored approach will help achieve the goals outlined in their NDIS plan.
Following the planning meeting, we will create a one-page profile to personalize their service and match them with the appropriate support staff and services. This process will enable us to provide the participant with the right level of support to achieve their goals.
Our Support Coordinator will regularly meet with the participant to assess their progress in achieving their NDIS goals. They will assist in navigating the NDIS by coordinating and connecting informal, mainstream, and funded supports in a service delivery environment. We are dedicated to walking alongside the participant throughout their NDIS journey and committed to helping them make their goals a reality.
Once we have established their supports and services, we will work together to monitor their progress towards their goals, and to ensure that their needs and aspirations are being met.
We will keep detailed records of their progress, which will provide valuable information when they review their plan with the NDIA on an annual basis.
Our aim is to help them develop the skills, capacity, and confidence to take control of their own NDIS plan. Over time, we will assist them to gradually take over some of the functions of the Case Coordinator, and teach them how to request the things they need.
We understand that this is an individual process, and everyone learns at their own pace. Once they are comfortable with the process, they will have the option to manage their NDIS plan independently. We are committed to helping them develop the skills and confidence they need to achieve their goals and take control of their own lives.
If there are any changes in the participant's needs or circumstances, the NDIS provider may conduct a review of the plan to make any necessary adjustments.
